URL parameters from Usergroup

During the May 2016 Usergroup, Paras Daryanani demoed how he had setup the URL to be www.xxxx.com/page?&Param1=123&Param2=456 into www.xxxx.com/page/123/456

Can someone explain how to do that in a XModPro friendly way?
