I have gotten very far with the steps outlined in the article here:

I am stuck at step 4, how to call the jquery in a template.  The example provides a method for modifying a skin file, but I would like to have this in a template instead, as the article states is possible.  

Does anyone have an working example with an autocomplete input box in a template?  

I just need something with the proper structure, using the example file fields and such would be ideal for me.
