Installation of the modules (xmod & formview) complete without error and you can drop the module on a page.

Attempts to activate license results in an OS level prompt from IIS wanting username/password. This is definitely coming from IIS and the server - not DNN.

Entering a server admin credentials finally results in a 404 error - resource not found.

All the portals in this install are functioning otherwise perfectly with a variety of modules.
